Lid Eversion

Those who have already had their practical will know that lid eversion is part of the ocular examination.
It is essential for contact lens dispensing, as it allow us to examine the tarsal conjunctival to see if there are any contraindication for contact lenses wear,

Instruction

  1. Ask the patient to look down.
  2. Gently grasp the patient's upper eyelashes and pull them out and down.
  3. Place the shaft of an applicator or tongue blade about 1 cm from the lid margin.
  4. Pull the lid upward using the applicator as a fulcrum to turn the lid "inside out." Do not press down on the eye itself.
  5. Pin the eyelid in this position by pressing the lashes against the eyebrow while you examine the palpebral conjuntiva.
  6. Ask the patient to blink several times to return the lid to normal.

Age Related Macular Degeneration

Age Related Macular Degeneration is one of the most common eye disease in the developed world.
This is a very good video explain what macular degeneration is in layman terms. 


Summary
2 types of age related macular degeneration 
  • wet type
  • dry type
Wet type (Neovascular/exudates) treatment - intraocular injection of "Avastin"
Patient with wet macular degeneration develop new blood vessels under the retina. This causes hemorrhage, swelling, and scar tissue but it can be treated with laser in some cases.
Dry type (Non-neovascular) treatment - supplements like antioxidants, lutein and zeaxanthin. 
Dry macular degeneration,
although more common, typically results in a less severe, more gradual loss of vision.
Amsler's Grid can be used to detect.
